Please come to the virtual appointment with a camera-ready appearance and home. You must be able to walk through your home with a mobile camera connected via our online meeting platform.

Payments
You may submit your payment using a personal credit or debit card. No refunds if you cancel less than 48 hours before your appointment or miss your appointment time. Medicare provides coverage for home safety assessments with a referral from your primary care physician. Request Safe Haven Home Assessments. You may also submit your assessment report and request reimbursement from your insurance provider using the HCPCS code of T1028 for home environment assessment.
